COVID-19 Testing And Protocol Agreement

In an effort to mitigate the spread of COVID-19, Affair Recovery has put in place safety rules and precautions. We require all in-person EMS Weekend participants to:

  1. Complete a PCR test the MONDAY before EMS Weekend.
    • Self administered PCR tests and at-home tests will not be accepted.
    • Antigen rapid test results will not be accepted.
    • PCR tests are also called molecular tests, viral RNA tests, and nucleic acid tests.
  2. Email your PCR test results by noon on the THURSDAY before EMS Weekend to support@hope-now.com. If you receive a positive COVID-19 result, please contact us immediately via email. We will attempt to reschedule your reservation for the next available in-person EMS Weekend OR arrange for you to attend the EMS Weekend virtually from your home.
  3. Have health screenings each morning including temperature checks.
  4. Practice social distancing by allowing six feet of space between themselves and others.

Here's what you need to know about PCR testing:

What is a PCR test? Also called molecular tests, viral RNA tests, and nucleic acid tests, PCR tests are used to detect genetic material of the coronavirus.

Where can you get a PCR test? Several major pharmacy chains now offer PCR testing at select locations, including CVS, Walgreens and Rite Aid. To schedule an appointment, you may have to complete an initial questionnaire. You can get tested for free through Curative at one of 10,000 participating locations. The Curative test swabs the mouth instead of nose.

How much does a PCR test cost? Right now, many sites are offering PCR testing free of charge to adults and children over the age of 3. For more accurate pricing information, please contact your insurance and the testing location of your choice.

How long does it take to get PCR results? On average, it can take anywhere from 24 to 48 hours (some locations say up to seven days) to receive PCR test results. These times will vary depending on the testing location. To ensure you get your results in time, please contact the testing location of your choice.

The safety of our EMS Weekend participants is our top priority. Informed Consent for PCR COVID-19 Testing

  • I have read the above, and I voluntarily agree to undergo a PCR COVID-19 test in accordance with this agreement.
  • I Agree to complete a PCR test the Monday before EMS Weekend.
  • I agree to email my PCR results to the Affair Recovery team by 12:00PM (noon) Central Time on the Thursday before attending EMS Weekend in person.
  • I agree that if I do not undergo a PCR test on Monday before EMS Weekend or I do not email my PCR results to Affair Recovery by 12:00pm (noon) Central Time the Thursday before EMS Weekend, I will not be allowed to participate in EMS Weekend, unless in rare circumstances, Affair Recovery has given me express written permission to still attend, at which point I agree to be tested by Affair Recovery staff upon arriving at the retreat facility and will be charged a $100 testing fee per test to my current credit card on file.
  • I agree that if I have a fever, symptoms, or test positive during EMS Weekend, I will be required to self-isolate, and will no longer be able to participate in-person.
  • I understand that there are risks associated with undergoing a diagnostic test for COVID-19, and I understand that there may be a potential for false positive or false negative test results.
  • I agree that I am not eligible for a refund if my spouse or I do not get PCR tested in the timeframe and method as outlined in this agreement.



COVID-19 Warning and Waiver

Warning. The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is reported to be extremely contagious. There is no proven treatment or cure for COVID-19. Evidence has shown that COVID-19 can cause serious and potentially life threatening illness and even death. In the event you decide to attend an Affair Recovery EMS weekend retreat in-person, Affair Recovery cannot prevent you from becoming exposed to, contracting, or spreading COVID-19 while attending the retreat or otherwise receiving in-person services from Affair Recovery. Additionally, due to the nature of the activities during the EMS retreat, Affair Recovery is not requiring participants to wear masks at the EMS retreat weekend. If you are at high risk for severe illness, please talk with your healthcare provider prior to attending EMS Weekend about whether you need to wear a mask or take other precautions.

Waiver and Release. I have read and understood the above warning concerning COVID-19. I hereby choose to accept the risk of contracting COVID-19 for myself in order to attend the EMS Weekend retreat and to enter the retreat facility. I release and waive my right to bring suit against Affair Recovery, LLC, the retreat facility, and their respective officers, directors, managers, officials, agents, employees, contractors or other representatives and any other participants in the EMS Weekend retreat in connection with exposure, infection, and/or spread of COVID-19 related to attending and participating in the EMS Weekend retreat. I understand that this waiver means I give up my right to bring any claims including for personal injuries, death, disease or property losses, or any other loss, including but not limited to claims of negligence, and I give up any claim I may have to seek damages, whether known or unknown.
